PHC (Primary Home Care)

This is a Medicaid waiver program offered by DSSW. Services under this program include:

Escort: Accompanying clients on trips to obtain medical diagnosis, treatment, or both. Does not include the direct transportation of the client by the attendant.

Housekeeping
Laundering
Shopping
Storing Purchased Items
Washing Dishes
Personal Care Assistance:
Bathing

Dressing
Grooming
Routine Hair and Skin Care
Preparing Meals
Feeding
Exercising
Self-administered Medication
Toileting
Transferring/Ambulating
