Home Health company seeking Bilingual Case Manager for
Temple Office
Position
Summary: The PAS Case Manager manages
ancillary programs (i.e., Star+PLUS, Primary Home Care, Family Care,
Community-Based Alternatives Program, MDCP and Private Duty/Personal
Assistance/Sitter Services) and provides personal care as needed to patients
and families in accordance with Agency policies and scope of practice.
Tasks:
• Supervises all Personal Assistance
Services/Community Care Program employees in the planning and coordination of
patient care in conjunction with the physician’s plan of care and/or the state
program’s (i.e., Texas Department of Aging and Disability Services (TDADS) or
Managed Care Organization (MCO) Individual Service Plan (ISP).
• Responsible for assisting with
billing of all ancillary program patients on a regular basis in accordance with
agency billing policies.
• Responsible for the coordination
of billing, scheduling, and office staff interaction.
• Collaborates with Human Resources
Manager to ensure that all staff files are completed in a timely manner and
reviewed at least annually.
• Ensures the programs are following
regulatory guidelines and agency policies.
• Collaborates with payroll
department to ensure that employees’ time records are accurate and submitted to
payroll in a timely manner.
• Completes supervisory visits
within regulatory time frame.
• Prepares supervisory notes on the
day of the visit and incorporates it into the clinical record in a timely
manner.
• Communicates with State program
and MCO Case workers and Service Coordinators by telephone regarding service
breaks and/or client issues in a timely manner.
• Ensures that quality staff are
hired, trained, and in-serviced as needed.
• Tracks unstaffed hours on a weekly
basis and documents hiring efforts.
• Assists the Administrator with
budget planning and implementation.
• Participates in case conferences,
in-services, meetings/committees, and other programs as needed.
• Assists with the management of
home health team attendants’ personnel and health folders as requested by Human
Resources Manager.
• Observes confidentiality policies
at all times.
• Other duties as assigned by the
Administrator and/or his/her designee.

and their families.
• Knowledge of home care rules and
regulations is required.
• Travel required for client
visits/hourly care and daily and community activities as necessary.
• Must have reliable transportation,
a valid driver’s license and agency-required auto liability insurance.
• Able to work in a fast-paced,
stressful environment.
• Able to work independently.
Includes time management skills in being able to manage one’s own time and the
time of others.
• Excellent personal organization
skills and personnel management skills in motivating, developing, and directing
people as they work and identifying the best people for the job.
• Excellent interpersonal skills,
including active listening skills in giving full attention to what other people
are saying, taking time to understand the points being made, asking questions
as appropriate, and not interrupting at inappropriate times.
• Able to communicate with health
care professionals, clients, families, and the general public.
• Verbal and written communication
skills are required in communicating effectively as appropriate for the needs
of the audience.
• Must be able to travel to current
or prospective patients’ homes.
Experience
and Education:
• PAS Program management experience
preferred.
• Minimum of two (2) years’
experience as a supervisor or 2 years of college courses.
• Willingness to work with patients,
TDADS Caseworkers and MCO Service Coordinators.
• Good communication and writing
skills.
• Basic computer skills knowledge
and experience: must demonstrate competence with office applications such as MS
Word and Excel, and have the ability to readily learn agency specific software.
• Must be able to type 35 wpm with
80% accuracy.
• Demonstrate an ability to
adequately read, write, and communicate in English; ability to also effectively
communicate in Spanish preferred.
