Monday – Friday 8am – 4pm
$17 - $18/hr DOE

The PAS Care Team Assistant I is responsible for the receipt of home care referrals and the scheduling of visits as prescribed in the plan of care. The individual maintains a current master visit list and assigns visits according to acuity, skills required, and staff availability.

Tasks:
• Serve as the communication center between physicians, staff members, and patients.
• Responsible for responding to inquiries about the Agency’s services and about home care in
general
• Receive and coordinate telephone/fax referrals for home care services.
• Schedules or reschedules visits when staff members are unable to make them as scheduled
• Collaborates with the Payroll department to ensure that employee’s time records are accurate
• Communicates with client’s program case workers via telephone or by using form 2067
regarding services breaks, utilization, and/or client issues in a timely manner
• Gather pertinent clinical, demographic, and payer information, and communicate this
information to appropriate persons to assure smooth transition into home care.
• Reschedule visits when staff members are unable to make them as scheduled.
• Maintain master schedule of all field staff visits, making changes as required or requested
by patients, physicians, PAS Supervisor, or staff.
• Prepare referral and statistical reports for PAS Supervisor and/or Administrator as assigned.
• Submit required reports within the assigned time frame.
• Participate in staff meetings and in-service education activities as assigned by the
PAS Supervisor and/or Administrator.
• Perform client care when required
• Travel required for client visits/hourly care as necessary
• Observe confidentiality policies at all times.
• Comply with federal, state, & company policies, procedures, and regulations.
• Perform other related duties as assigned.

• Excellent interpersonal relations skills, including active listening skills such as giving full

• Willingness to work with health care professionals, clients, families, and the general public.
• Written and verbal communication skills are required to communicate effectively as
appropriate to the needs of the audience.
• Must be able to read, write and comprehend English; must be able to communicate efficiently
in English.
Experience and Education:
PAS Program management experience preferred
Minimum of two (2) years’ experience as a supervisor or 2 years of college courses
Willingness to work with patients, HHS Caseworkers and MCO Service Coordinators
• Six (6) months to one (1) year previous staffing experience in a home health agency or other
medical setting, preferred
• Home Health experience strongly preferred.
• Knowledge of home care rules and regulations required.
• Must be able to demonstrate competence with computer applications such as MS Word and
Excel and have the ability to readily learn agency specific software.
• Must be able to type 35 wpm with 80% accuracy and 80% accuracy in Excel.
• Must have reliable transportation, a valid driver’s license and auto liability insurance.
Physical Requirements:
• Ability to travel to client’s homes for visits/hourly care and to (community) (daily) activities.
